WACHS Midwest Regional Renal Team
The WA Country Health Service Midwest Regional Renal team provides a single point of contact for patients with kidney disease across the Midwest region, delivering care coordination and assistance to patients navigating their kidney disease journey. This is achieved through face-to-face consults, home visits, telehealth, and telephone support. Team members focus on early detection, primary and secondary prevention, and management of chronic kidney disease. The Renal team also assist patients in accessing specialist services, pre-dialysis education, vascular access support, and home therapies through establishing strong relationships with local health professionals and organizations to ensure patients receive optimal care close to home. The Midwest Regional Renal Team serve over 730 patients across 600,000 square kilometres and travelled over 10,000 km in 2023/24, providing 2,300 occasions of service. They also facilitated a visiting kidney transplant team, with six successful transplants to date. To ensure they are all up to date with contemporary knowledge, the team actively seek education and training opportunities whenever available.

Community Health in a Virtual Environment (Co-HIVE) Aged Care Service
The Co-HIVE Nursing Team is comprised of specialised nursing clinicians including Clinical Nurse Consultants and Clinical Nurses who work collaboratively with Geriatricians to care for older adult consumers with an emphasis on acute deterioration and/or complex conditions. The team is essential in facilitating gold standard care for elders by working with care providers across the sector to reduce care barriers. They achieve excellence by continuously identifying opportunities for improvement and promoting a collaborative culture through upskilling and educating residential aged care facility staff, leading to improved clinical care that aligns with resident goals. Enhancing patient, family, and multidisciplinary involvement results in better patient outcomes and is seen through the team’s holistic approach to geriatric nursing which focuses on educating and supporting patients and families, enabling access to community services and better care in their own environment. The introduction of virtual care services has further improved patient satisfaction and care quality, demonstrating the team’s commitment to proactive and convenient care for older adults. The team is most proud of enhancing patient, family, and multidisciplinary involvement for better patient outcome.

Derby Maternity Midwifery Team
The Derby maternity midwifery team provides exceptional clinical support for women, babies, and families in Derby, Fitzroy Crossing, and surrounding communities. They offer comprehensive care for antenatal, post-natal, labour, and delivery along with education for women and staff. The team prioritizes quality and patient safety ensuring high standards through preceptorship, orientation, and supervision. Their holistic, patient-centered approach fosters a supportive and culturally safe environment, empowering expectant mothers with knowledge and confidence. The team’s dedication extends beyond clinical care by actively engaging with local healthcare providers to ensure accessibility and comprehensive support. Their compassion, empathy, and exemplary communication skills create a trusting and safe environment for patients. The team actively engage and collaborate with other local healthcare providers to create a cohesive network of support not only in Derby, but also through surrounding communities demonstrating their commitment to ensuring every woman has access to quality midwifery care. The team feels privileged to work alongside local Indigenous women and their families, building relationships based on trust, respect, and a deep understanding of culture and individual needs. The team’s proudest moment has been leading three well attended community events that focused on providing holistic, safe, culturally appropriate, women and family centred care.

Endoscopy Unit Peel Health Campus SMHS
The Peel Health Campus (PHC) Endoscopy unit exemplifies excellence in person-centred care consistently demonstrating the South Metropolitan Health Service (SMHS) core value of teamwork. The unit, including clinical nursing, endoscopy bookings and ancillary staff, receives numerous compliments from patients for their welcoming, safe, and supportive environment. The team is proud and passionate about their work, treating all patients like family and making a difference in their lives. Despite the fast-paced nature of their work, the team manages to complete an average of 15 procedures daily, providing a six-day service. Recently, they implemented strategies to prioritise patients over boundary for their wait time category, increasing to a seven-day service and taking on extra shifts. The Senior Nursing team led by example, maintaining high standards and patient-centered care. This collaborative approach, demonstrating resilience and dedication has resulted in a significant decrease in waitlist times. The team’s proudest moments are ensuring patients receive good care, whether it’s by bringing nursing skills from overseas and integrating into a team, returning to nursing after 14 years and still enjoying it, continuing to nurse with passion after 35 years, or completing a nursing degree and starting a career.
